REQUESTING BIRTH AND DEATH RECORDS:

The Division of Vital Statistics can issue a certified copy of a birth certificate of any citizen who was born in the State of Ohio. The Division can also make a certified copy of a death certificate of any citizen who died in the City of Lakewood.

Beginning January 1st, 2025, this office will be able to issue death certificates for any Ohio death occurring in the year 2025. For all previous years you must contact the city where the event took place.

Birth and death records can be obtained by calling (216) 529-7690 Monday through Friday from 9:00 a.m. to 4:00 p.m.

Walk ins welcome, no appointment necessary.

Please note that a required fee of $25.00 is charged for every copy of birth and death records certified.  Forms of payments accepted are credit/debit cards, checks, money order and cash. ID is required if paying by card or check. We do not accept Apple Pay or Tap payments.

When a certificate is requested via email or fax a customer service representative will call the phone number provided for payment. No Certificate can be delivered by email or fax.

To obtain these certificates, download our fillable application online and return the completed copy by mail, e-mail, fax or walk in. Applications are available in the office.  NOTE: You MUST open the application in Adobe Reader for the fill-in fields to work properly.

The Division of Vital Statistics does offer paternity affidavits in the office.

Fees:
In accordance with section 3705.24 of the Ohio Revised Code, we are required by law to charge a fee for each certified copy of a vital record issued. The fee at this office for each certified copy of a birth, death, or fetal death record is $25.00.
